Unser Technologiepartner Direct Conversion hat das Application Innovation Centre (AiC) in Gräfelfing, München eröffnet und dort am 5/6. Juli 2019 sein 1. Internationales Symposium zu „Photon Counting Technologies & Applications“ abgehalten.

AB-CT fühlt sich geehrt, als Gastredner eingeladen worden zu sein und den Brust-CT-Scanner nu:view einem hochkarätigen Publikum vorstellen zu können. nu:view verwendet einen Single-Photon-Counting Röntgendetektor – eine gemeinsame Entwicklung von AB-CT und Direct Conversion. Dieser Detektor ermöglicht eine sehr hohe Bildauflösung bei gleichzeitig niedriger Strahlendosis.

Willi Kalender, Gründer und wissenschaftlicher Berater von AB-CT, hielt einen Ehrenvortrag über die historische Entwicklung der Computertomographie in den letzten drei Jahrzehnten. CTO Daniel Kolditz teilte sein Expertenwissen aus technologischer Sicht.

Abschließend berichtete Prof. Dr. Boss, Chefarzt am Universitätsspital Zürich und erster Anwender von nu:view, über seine Erfahrungen der medizinischen Anwendung dieser disruptiven Technologie.

Im AiC-Showroom konnten sich die Interessenten ein Bild vom nu:view-Modell in Echtgröße machen.

Abstract:

Technical overview of the photon-counting breast CT system and its performance, the first clinical patient results and an outlook of potential applications.

Photon-counting spiral breast computed tomography (BCT) is a novel approach in breast imaging. It provides high resolution at very low dose using novel direct-conversion photon-counting detector technology combined with a spiral CT acquisition concept. The system (type nu:view, AB-CT – Advanced Breast-CT GmbH, Erlangen, Germany) presented here has received CE mark in 2018 and is available in clinical use. Performance tests showed a superior image quality compared to mammography, breast tomosynthesis, clinical CT as well as cone-beam breast CT systems based on conventional indirect converting X-ray detectors. The first clinical results confirm that the superposition-free 3D images are optimal for soft-tissue differentiation and micro-calcification characterization and this at dose level of mammography.

A dedicated breast CT scanner that features a photon-counting detector produces high-quality images at a low radiation dose. The findings indicate that the CT scanner could be a useful tool for breast cancer screening, according to a study published online March 2 in Investigative Radiology.

In fact, a team from the University of Zurich in Switzerland believes that breast CT has the potential to further improve breast lesion detection and characterization compared with mammography and digital breast tomosynthesis — particularly because it does not require breast compression, and the photon-counting detector technology reduces radiation dose to levels comparable to those of a conventional mammogram.

A dedicated breast CT scanner that features a photon-counting detector produces high-quality images at a low radiation dose. The findings indicate that the CT scanner could be a useful tool for breast cancer screening, according to a study published online March 2 in Investigative Radiology.

In fact, a team from the University of Zurich in Switzerland believes that breast CT has the potential to further improve breast lesion detection and characterization compared with mammography and digital breast tomosynthesis — particularly because it does not require breast compression, and the photon-counting detector technology reduces radiation dose to levels comparable to those of a conventional mammogram.

„Previous studies investigating the use of conebeam breast CT in comparison to mammography reported higher radiation doses for breast CT, especially when referring to a screening setting,“ wrote a group led by Dr. Nicole Berger. „More recently, new strategies have strongly increased the dose efficiency due to the use of novel photon-counting detector technology, resulting in dose levels for breast CT comparable to those of screening mammography or lower.“

Yet another benefit to breast CT is that it can be used with contrast, Berger and colleagues wrote.

„The possibility to perform contrast-enhanced examinations indicates a possible usage of breast CT comparable to breast MR imaging with the additional capability to simultaneously detect microcalcifications and soft-tissue contrast enhancement,“ the researchers noted.

Holy grail?

Researchers continue to seek ways to screen for breast cancer that will minimize potential harms and maximize benefits, and Berger’s team is no different. This study used data from start-up firm Advanced Breast-CT’s dedicated spiral breast CT system, nu:view, which has a photon-counting detector based on cadmium telluride (CdTe) crystals. It’s the first clinical in vivo application of nu:view, which received CE Mark certification in September of last year.

Questions have arisen as to whether the resolution of breast CT is high enough to depict microcalcifications. The new detectors address this concern.

„Up to now, resolution of breast CT did not reach high enough resolution to visualize microcalcifications, which the new dedicated breast CT can provide. … The high resolution, but also the possibility to evaluate the images in different planes, increases and facilitates the characterization of lesions regarding suspicious features such as conspicuity, calcifications, and asymmetries, as well as masses,“ the researchers wrote.

For the current study, 12 women referred for breast cancer screening underwent bilateral breast CT in the prone position as well as breast ultrasound. Three of the 12 women had previous mammography available for comparison.

Two radiologists interpreted 15 breast CT exams, which they evaluated for image quality using the following measures: spatial resolution, contrast resolution, signal-to-noise ratio, and contrast-to-noise ratio. The researchers used final reports from the breast CT evaluations with ultrasound assessment as the reference standard.

Finally, study participants gave feedback about both positioning and overall comfort during the exam using a scale of 1 to 5 (with 5 equal to maximum comfort).

The major pectoral muscle was included in 15 breast CT scans (62.5%); a glandular component was partially missing in two (8.3%) of the 24 scanned breasts. Berger’s group found a „ring“ artifact in all scans, although it did not influence image interpretations.

Breast CT identified three masses that were confirmed to be simple cysts on ultrasound. Ultrasound identified another five simple cysts and two solid benign lesions. In all of the breast CT exams, 12 calcifications were identified, with a median size of 1.1 mm; both readers identified all three masses and 11 of 12 calcifications on breast CT.

As for the women’s feedback about the comfort of the exam, the majority (75%) ranked it a 4 on a scale of 1 to 5; all women with prior mammography ranked the breast CT exam as more comfortable due to the absence of compression.

With respect to radiation dose, Monte Carlo simulations on phantoms found that mean dose was in the range of 5.08 mGy. For point of comparison, Hendrick et al in 2010 found radiation dose from two-view digital mammography of 3.7 mGy and for film-screen mammography of 4.7 mGy.

„The new dedicated spiral breast CT equipped with a photon-counting detector provides high-quality images at low radiation dose and shows considerable potential for screening and diagnosis of breast cancer, along with improved patient comfort compared with conventional mammography,“ Berger and colleagues concluded.

The EUSOBI Mammography & beyond Course 2019 aims to give a thorough overview of different Mammography (MG) based techniques like tomosynthesis, contrast enhanced MG, synthetic MG and CT of the breast. Beside fundamental technical considerations, results of different ongoing clinical trials will be presented and training on different MG based techniques will be provided.

The course will take place in Vienna, Austria, from February 25-26, 2019.

Theoretical lectures will be enhanced by hands-on workshops on systems provided by different vendors.

According to the German Cancer Research Center in Heidelberg, some 69,000 women in Germany are diagnosed with breast cancer every year, with almost 18,000 dying of the disease. Conventional diagnosis methods, while well established, are not always reliable. 3D imaging with high isotropic resolution, on the other hand, offers clear advantages.

Conventional diagnosis methods, while well established, are not always reliable. 3D imaging with high isotropic resolution, on the other hand, offers clear advantages.

nu:view, the world’s first breast CT scanner to use spiral CT technology, is the brainchild of Erlangen-based company AB-CT. With the CE marking for nu:view in place, the new scanner is already in use on patients at the University Hospital Zurich (USZ).

What sets nu:view apart is the very high image resolution coupled with low radiation dose and short scan times, explains AB-CT CEO Benjamin Kalender, referring to the benefits of the new CT scanner. “To obtain the best possible image quality and maximize radiation dose efficiency at the same time, the detector uses state-of-the-art single photon counting technology.” Unlike conventional scintillation, nu:view uses detectors made of cadmium telluride (CdTe), transforming every x-ray photon directly into an electric pulse.

In a single rotation around the female breast, 2,000 projection images are created, with a full scan taking as little as seven to 12 seconds—at very low radiation levels

without breast compression, ensuring excellent patient comfort. “For the first time we have a CT scanner that makes it possible to acquire an image of the entire female breast in a single scan in true 3D, imaging both the soft tissue and the calcifications,” the CEO explains.

Prof. Dr. Andreas Boss, senior consultant for breast imaging at the Institute for Diagnostic and Interventional Radiology at University Hospital Zurich and first user of nu:view, can attest to its merits: “The images are impressive; the image quality is excellent. Compared to mammography, the three dimensional, non-superimposed images make it far easier to detect micro-calcifications. This non-compressive method also means less discomfort for the patient and reduces the number of additional ultrasound images into the bargain.”
